Claim Entry Application

Once logged into the website, an on-line Data Entry Application is available under the "Claim Management" tab. The Claim Management tab may also be used to upload supporting documentation and view existing claims. For help getting started, click here.

Database Conversions

If your claim information is already in a database, you may want to convert your data rather than re-enter it. Once logged in, there is further information for converting existing data on the Downloads Page
